COVID-19 Vaccination Web Portal
*** DO NOT complete this form if you have already had your first dose. You will be contacted by Nash County for your second dose.
*** You must be in Group 1 - 3: School & Childcare Workers to be placed on the waitlist. Your information will be deleted from the list if you are not eligible for the vaccine yet.
*** If you have already been added to the waitlist by calling the Nash County Senior Center or the Nash County Health Department, DO NOT start this process. It will duplicate your information in the system.
Step 1 - Complete the Nash County COVID-19 Vaccination Registration Form. This adds your information to our waitlist, getting you a spot in line to get your shot! The information form this form is also entered into the state's registration system at the end of each day. (Please use a separate email for each individual person that you complete the form for.)
Step 2 - It may take a few days, but you will receive an email from the state's registration system asking you to complete their questionnaire. Completing this questionnaire online will speed up the process at your vaccination appointment. If you can't complete it online, find the link to the questionnaire below, complete it, and bring it to your appointment. Staff will also be able to help you complete the questionnaire at your appointment.
Step 3 - Be patient. There is a high demand for the COVID-19 vaccination and limited supply.
Step 4 - You will be contacted through the email you provide when it is your turn on the waitlist. You must also be eligible to receive the vaccine based on your group. Currently, we are on groups 1-3: School & Childcare Workes, which includes people 65 or older, first responders, those in the medical community, and school/childcare workers. The email will let you select an appointment time on the date of our next available clinic. After you finish your appointment selection, you will receive an email confirmation. If you enter a cell phone number during this process, you will receive a reminder text before your appointment.
Step 5 - Show up to your appointment and get vaccinated!
Step 6 - When it's time for your second dose, you will receive another email to schedule your second-dose appointment.

Once we are finished with the waitlist, our online portal will transition where users can schedule their own appointments from a list of available dates and times.
