Registration - Help Speed Up The Vaccination Process

To help speed up the process, please fill out this registration form and bring it with you to the vaccination event you are attending. Print and complete our COVID-19 Vaccine Questionnaire.

Nash County is currently in PHASE 1B - Group 1

Beginning Thursday, January 7th, Nash County moved to Phase 1B-Group 1. This moves opened the vaccine up to residents who are 75 years or older. Everyone will be able to access the vaccine in phase 4. For more information about the COVID-19 vaccine, please call the Nash County Health Department at 252-459-9819.
