Who can receive home health services?
Home health patients are often recovering from illnesses or accidents or they may be suffering from chronic, long-term conditions such as a stoke, respiratory conditions, diabetes; joint replacements or high-blood pressure.

To request home health services; you or your family may ask your physician to refer you for these services. If you are in the hospital, the discharger planner may refer you for home health services.
Anyone can receive home health Services who:

  • Lives in (or within 10 miles of) Nash County

  • Is under the care of a physician who will order services

  • Is homebound

  • Requires intermittent skilled services
