Nash County Health Department offers a Family Planning program that is designed to enhance the reproductive health of our female patients and their families.

What to Bring to Your Appointment:

  • Proof of ID
  • Proof of household income (tax return or pay stub)
  • Insurance card


Who is Eligible for Family Planning Medicaid?

  • U.S. Citizen or Specific Lawfully Present Immigrants
  • Any Gender
  • Resident of North Carolina
  • Income 195% or less of the Federal Poverty Level
  • Any Age (including teens)

Not Eligible:

  • People who have had a sterilization procedure (i.e., vasectomy, hysterectomy, and tubal ligation ("tubes tied")). If you have had a hysterectomy or tubal ligation and you are looking for services, please check out our Women's Health Watch program.

Apply online at https://epass.nc.gov/ or in person at the Department of Social Services.

The Nash County Health Department accepts a variety of insurance providers. Please look under our "clinical services" page to view fees and in-network providers. All services can be provided through a sliding scale fee, meaning the cost is income-based.
